SK Lung and Sleep collects information from you, solely for the purpose of providing appropriate and effective medical care. We only request information that we feel will help in investigating, diagnosing and aiding in any treatment to better your overall health needs.

We are dedicated to protecting your privacy and managing your personal information in a responsible manner in accordance with the Privacy Act 1988 (Cth), the Privacy Amendment (Enhancing Privacy Protection) Act 2012, the Australian Privacy Principles and relevant State and Territory privacy legislation (referred to as privacy legislation).

GATHERING OF DATA:

We aim to gather data that is necessary and crucial to provide you with the proper medical care. This information may include your personal details such as name, date of birth, address, gender, medical and family history. This information is usually stored on our medical software which may be linked to a cloud database. We aim to minimise handwritten records.

We may also find it useful to gather information from other sources such as other or previous treating specialists, pathology and radiology companies and My Health Record system.

We may be required by law to retain medical records for certain duration of time depending on your age at the time we provide services.

DATA USEAGE AND DISCLOSURE:

Your personal information is treated with the upmost respect and in a private and confidential manner. Our aim is to use or disclose your information only when it relates to your care and management, or when it is reasonable to use this information for ongoing care and management.

There may be certain situations or circumstances when we may be permitted or required by law to disclose your information to third parties. Some examples, include: Medicare, law enforcement agencies (i.e. Police), government regulatory bodies, courts of law, tribunals, debt collection agents and/or to the My Health Record System.

We may also disclose data about yourself to outside contractors (i.e. IT service provider) to carry out tasks on our behalf. Outside contractors are required to only use information about you for  the sole purpose of completing  the task we have requested of them.

DATA QUALITY AND SECURITY:

The data provided to us will be managed so that it is kept accurate, relevant and up to date. As a result, we may ask you to confirm your personal data prior to your consultation. It is crucial that we are able to contact you so that we are able to provide you with the appropriate medical care.

We aim to protect your personal data using a number of steps including: securing our premises, using complex passwords and two factor authentication for electronic medical records and removing paper copies of your information via shredding with confidential disposal.

DATA ACCESS:

You are entitled to request access to your medical information. We request that this is put in writing and we will aim to respond to this withing a reasonable time period. There may be a small administrative fee attached to retrieving and providing you with copies of your medical records.

You may be denied access to your medical records in rare circumstances as permitted by the law – e.g. if disclosure has the potential to cause serious threat to your health or safety. If this was to occur, we would inform you of such and liaise with you to discuss your options moving forward.
